Insurance

HealthWise Resources Physical Therapy is a provider for Blue Cross – Blue Shield/Wellmark Insurance. No prescription is needed for treatment. Coverage is never guaranteed. Typically, 20 visits per year are covered by insurance.

Any patient who has Medicare or any other kind of insurance receives a discounted rate of $70.00 per treatment if payment is given at the time of treatment. Whenever possible a superbill with the codes on it is provided to the patient for them to file for partial or full reimbursement with their insurance. Reimbursement is never guaranteed.

We are not a provider for Workers Compensation or Medicare..

Payment may be made by cash, check, Visa, or MasterCard.
